You may want to check that your test kit is giving accurate results.

Post your results here including temperature plus other details such as species of fish, size and number of fish. A description of your system including amount of grow bed volume would be handy as well.

What are your water temps like and what fish are you keeping? Pics of the system? If you are able to measure DO directly that would be helpful, but I can probably tell you if you have enough aeration by looking at how it's built.

Also, any symptoms that your fish are displaying prior to death? That can help narrow it down.
